You are simply viewing the same content, one single file or photo, in two different ways and mistaking that for a duplicate file or photo. But very often these aren't duplicates at all. That kind of thing and you end up with items. You may have done something to duplicate the file or maybe sync it twice from a camera. Sometimes, of course, these really are duplicates.
